I do not know regex, can I use it?

Yes. Plain text rules (old => new) are enough; regex is optional.

Do channels get deleted when I delete a group?

Yes; 'Groups to delete' removes the channels in that group from the list. To only empty the group you can rename it to blank.

What happens to #EXTGRP lines?

For channels whose group changes, the group-title attribute is written and the old #EXTGRP line is removed.
